New patient paperwork and registration will be texted or emailed to you prior to your appointment.
Bring the following to the first appointment:
- a copy of your child’s vaccination records
- a copy of your driver’s license and insurance card
Newborns
If you are expecting a newborn you can call to schedule a Newborn Well appointment after the birth of your child.
Kids First Pediatrics has privileges at Flower Mound Presbyterian Hospital. Please let the hospital staff know that your child is a Kids First kid and they will contact our doctor on call.
When providing insurance information, newborns are typically covered under the Mother’s insurance for the first month.
