Preventing Medical Errors

Medical errors carry the potential for harm. Minimizing the risk of a potential error requires a team approach. Errors can occur at any place including hospitals, clinics, pharmacies or even your home. It can involve medications, surgery or laboratory reporting.

How can you protect yourself and your family?

Make sure your provider knows all medications that your child is taking, including over the counter medicines and herbal remedies.

Make sure that any allergic reaction your child has had is documented in the chart and at the pharmacy

Ask for information about any medicines you are prescribed. Make sure you understand what it does, how you take it, potential side effects and interactions between other medications.
